虽然肩以上是有菌区，但是这个指的是医生自己的肩和头部是有菌的，并没有说手超过肩就是有菌。现在手术室都是层流，百级层流手术室，一立方米空气中灰尘不得超过一百个。所以，医生手举过头，只要没有碰到有菌区，其实跟从一个无菌区跨越到另一个无菌区是一样的，完全没有违反无菌原则。
